The COVID Public Health Emergency (PHE) has ended. Medicaid needs your current contact information to keep you in their system and to keep your coverage up-to-date.

If you have Medicaid or CHP+, YOU MUST UPDATE YOUR CONTACT INFORMATION TO AVOID A GAP IN COVERAGE.

Don’t miss important notices about your Medicaid coverage. Update your information now:

• Visit Colorado.gov/PEAK

• Use the Health First Colorado app on your phone. This free app is for Health First Colorado and CHP+ members.

• CHP+ members can call 800-359-1991 (State Relay: 711).

• Visit Mesa County Department of Human Services or call (970) 241-8480
